We hope you all have had a restful and restorative summer break thus far!
This is your gentle reminder to please submit your students’ updated health certificates (with updated immunization records attached) and dental forms! You can find the DC Department of Health forms here:
All forms expire after a year from the date of exam; all students need updated forms each year! If you submitted updated forms from last spring or late fall, please simply submit updated forms whenever they have their routine appointments!
Important: DC will be implementing “No Shots, No School” so please ensure your child’s records are all up to date! I will also be reaching out to families who are showing as “non-compliant” in the system!
If your student requires any type of medication at school (even over the counter medications and topicals), please have your provider fill out a medication form here: Medication Form
If your student requires an action plan for asthma, anaphylaxis, etc., please find them here:
Also, please don’t hesitate to reach out to me about any health issues or concerns your student may have that may affect them while at school. We have a lot of students here, and if the health concern is not listed on their health certificate, I am not always aware of it!
